Intensity Modulated Radiotherapy (IMRT) Versus Helical Tomotherapy (HT) in Esophageal Cancer: A Comparative Analysis

Purpose/Objectives: Definitive chemoradiation is an accepted alternative treatment in the management of esophageal cancer patients who are not candidates for surgery. The location esophagus poses a challenge because its close proximity to critical organs. Various modern techniques radiation planning offer advantage better dose conformality and increased sparing organs at risk. This study aims compare dosimetric parameters Intensity Modulated Radiotherapy (IMRT) Helical Tomotherapy (HT) radical radiotherapy patients. Materials/Methods: A total 38 squamous cell carcinoma with clinical staging cT2-T4/N0-N2/M0 planned definitive were enrolled from June 2021 till November 2022. CT simulation was done intravenous oral contrast, contouring tumor volumes risk according RTOG guidelines. All 50.4Gy 28 fractions. Radiation plans IMRT HT generated each patient. Dosimetric PTV D98, D2, Dmean, V95, V110, Homogeneity Index (HI) Conformity (CI) noted both plans. Organs (OAR) such as Lung (V20, V30, V5, Dmean), Heart (V25, Spinal Cord Dmax, Larynx (Dmax, Dmean) Liver Dmean noted. two different compared analyzed using SPSS software v23 paired t-test.Results: coverage similar terms V95 D98. regions receiving more than 107% 110% prescribed significantly higher HT, on assessing V107, V110 D2 (p=0.001).
